dc.description.abstract | Alzheimer's disease is a progressive and irreversible neurodegenerative condition with development characterized by asymmetrical brain atrophies. Across life, due to neuronal aging, a cognitively healthy brain presents structural changes. However, with the development of the Alzheimer's, these changes are more accentuated, being less intensive in the prodromal stage of Alzheimer's, known as mild cognitive impairment, and more pronounced as the disease progresses. In this research, we developed three approaches to aid in diagnosing patients with mild cognitive impairment and mild Alzheimer's disease, each of which analyzed distinct and complementary properties related to the disease, namely asymmetry, atrophy, and biological brain aging. In addition, we combined and tested the approaches to measure impact on outcomes. In the analysis of structural asymmetries, we extracted multi-scale attributes of the hippocampal regions and created an asymmetry index. We obtained classification results comparable to other studies focused on the hippocampal regions, and the created asymmetry index showed statistically significant results consistent with the medical literature. In the analysis of structural atrophies, we automatically selected and classify discriminative landmark points between populations. Next, we classify the images based on a quantitative assessment obtained from the results of the landmark points. The obtained image classification results surpassed many studies published in the literature and are comparable to others. In the analysis of the biological aging of the brain, we developed an age estimation model using deep learning and images of cognitively normal patients with the same age range as patients with mild cognitive impairment and mild Alzheimer's disease. The age estimation error between the groups showed statistically significant results and presented a significant correlation with the Mini-Mental State Examination clinical test. The estimation results were competitive among existing studies, but the classification results were below expectations. Finally, combining the first two approaches brought positive contributions to the results, with up to 3% gains in AUC. | eng |

dc.description.resumo | A doença de Alzheimer é uma condição neurodegenerativa progressiva e irreversível com desenvolvimento caracterizado por atrofias cerebrais com desenvolvimento assimétrico. Ao longo da vida, um cérebro cognitivamente saudável apresenta mudanças estruturais, no entanto, com o desenvolvimento do Alzheimer, essas mudanças são mais aceleradas, com menor intensidade no estágio de comprometimento cognitivo leve, e mais acentuada a medida que a doença progride. Neste projeto de pesquisa foram desenvolvidas três abordagens para auxiliar o diagnóstico de pacientes com comprometimento cognitivo leve e doença de Alzheimer leve, sendo que cada uma analisou propriedades distintas e complementares relacionadas à doença, a nomear a assimetria, a atrofia e o envelhecimento biológico cerebral, por fim, as abordagens foram combinadas para medir o impacto nos resultados. Na análise de assimetrias foram extraídos atributos multi-escala das regiões hipocampais e criado de um índice de assimetria. Os resultados de classificação foram comparáveis a outros estudos focados nas regiões hipocampais e o índice de assimetria criado apresentou resultados estatisticamente significativos e consistentes com a literatura médica. Na análise de atrofias, pontos salientes discriminativos entre as populações foram automaticamente selecionados e posteriormente classificados. A seguir, as imagens foram classificadas com base em uma avaliação quantitativa obtida a partir dos resultados dos pontos. Os resultados de classificação superaram muitos estudos publicados na literatura e são comparáveis a outros. Na análise do envelhecimento biológico do cérebro foi desenvolvido um modelo de estimação etária utilizando aprendizado profundo e imagens de pacientes cognitivamente normais com a mesma faixa etária de pacientes com declínio cognitivo. O erro de estimação etária entre os grupos apresentou resultados estatisticamente significativos, além de apresentar correlação significativa com o teste clínico Mini-Mental State Examination. Os resultados foram competitivos entre os estudos já existentes. Por fim, a combinação das duas primeiras abordagens trouxeram contribuições positivas para os resultados, com ganhos de até 3% na AUC. | por |
