Segurança da criança hospitalizada em unidade de terapia intensiva: compreensão de pais e enfermeiros
Abstract
Researches on patient safety thematic have shown significant development.
Nevertheless, it is still necessary to strengthen family involvement, which is essential
for the security of hospitalized children. In this study, therefore, the objective was to
develop a representative theoretical model of the insertion of parents in the security
of hospitalized children, in the view of nurses and parents, being a qualitative,
comprehensive research conducted in a tertiary hospital in the state of Paraná. The
following references were selected: Symbolic Interactionism (theoretical) and
Grounded Theory (methodological), both used for data collection, organization and
analysis. The participants were recruited from the theoretical sampling in the
neonatal and pediatric intensive care units, both references in the state under study.
Data collection took place between January 2018 and December 2019, through
audio-recorded semi-structured interviews and conversation circles, with the
participation of 25 parents and 18 nurses working in the care units. The research was
initiated after approval by the Human Research Ethics Committee. The process of
analysis of the collected data obtained as a central theoretical model “seeking safe
care and without harm”, which was divided into three categories (Experiencing the
hospitalization of a child in ICU; (In)safety of parents of children in ICU; Facing
complex and multifactorial processes) and their subcategories. Parents initially
experience difficulties in the process of adaptation to the intensive environment,
requiring the support of family, professionals, peers and spirituality. After the initial
phase, they recognize the possibility of child care errors, and raise concerns about
the child’s long-term quality of life. Parents recognize in the physical presence a
determining factor for the inclusion in the child’s safety, since from this they can know
their child, recognize warning signs, act in favor of their child’s safety and question
professionals. Although the presence is decisive, it is necessary that professionals
and family members establish partnership relations for this involvement to take place.
Performing the care is recognized as an insertion aid tool. The presence of protocols
and humanization are physical and organizational aspects that also interfere with the
insertion of parents in child care and safety. Communication is valued, although full
of challenges, such as the pursuit for the ideal moment to establish the relationship
between parents and professionals and the transfer of information. Challenges such
as workload, staff sizing, care sharing, vocational training and multiprofessional work
influence the effectiveness of parents’ insertion in the safety of hospitalized children.
It is concluded that parents and professionals seek to provide safe and without harm
in care to children hospitalized in intensive care units. Inserting parents in the safety
context is a complex factor and influenced by many variables, but, when effective,
brings benefits to the relationship between professionals and parents, as well as
reflexes in the improvement of child safety. Investigaciones sobre seguridad del paciente han mostrado un desarrollo
significativo. Sin embargo, todavía es necesario fortalecer la participación familiar,
que es esencial para la seguridad de niños hospitalizados. En este estudio, por lo
tanto, el objetivo fue desarrollar un modelo teórico representativo de la inserción de
los padres en la seguridad de los niños hospitalizados, desde la perspectiva de las
enfermeras y de los padres, siendo una investigación cualitativa, integral, realizada
en un hospital terciario en el estado de Paraná. Fueron seleccionadas las siguientes
referencias: Interaccionismo Simbólico (teórico) y Teoría Fundamentada en Datos
(metodológica), ambas utilizadas para la recopilación, organización y análisis de
datos. Los participantes fueron reclutados a partir del muestreo teórico en las
unidades de cuidados intensivos neonatales y pediátricos, ambas referencias en el
estado del estudio. La recopilación de datos tuvo lugar entre enero de 2018 y
diciembre de 2019, a través de entrevistas semiestructuradas grabadas en audio y
ruedas de conversación, con la participación de 25 padres y 18 enfermeras que
trabajan en las unidades de atención. La investigación se inició después de la
aprobación del Comité de Ética de Investigación Humana. El proceso de análisis de
los datos recopilados obtenidos como un modelo teórico central “buscando atención
segura sin dañar”, que se dividió en tres categorías (Experimentar la hospitalización
de un niño en la UCI; (In)seguridad de los padres de niños en UCI; Enfrentando
procesos complejos y multifactoriales) y sus subcategorías. Inicialmente, los padres
experimentan dificultades en el proceso de adaptación al ambiente intensivo, lo que
requiere el apoyo de la familia, de los profesionales, de los compañeros y de la
espiritualidad. Después de la fase inicial, reconocen la posibilidad de errores
relacionados con el cuidado infantil y expresan inquietudes sobre la calidad de vida
del niño a largo plazo. Los padres reconocen en la presencia física un factor
determinante para su inclusión en la seguridad del niño, ya que a partir de esto
pueden conocer a su hijo, reconocer las señales de atención, actuar a favor de su
seguridad y cuestionar a los profesionales. Aunque la presencia es decisiva, es
necesario que los profesionales y los miembros de la familia establezcan relaciones
de asociación para que esta participación tenga efecto. La precaución se reconoce
como una herramienta de ayuda para la inserción. La presencia de protocolos y la
humanización son aspectos físicos y organizativos que también interfieren con la
inserción de los padres en el cuidado y la seguridad de los niños. La comunicación
es valorada, aunque llena de desafíos, como la búsqueda del momento ideal para
establecer la relación entre padres y profesionales y la transferencia de información.
Desafíos como la carga de trabajo, el tamaño del personal, la atención compartida,
la formación profesional y el trabajo multiprofesional influyen en la efectividad de la
inserción de los padres en la seguridad de los niños hospitalizados. Se concluye que
los padres y los profesionales buscan brindar atención segura y sin secuelas a los
niños hospitalizados en unidades de cuidados intensivos. Insertar a los padres en la
seguridad es un factor complejo e influido por muchas variables, pero, cuando es
efectivo, aporta beneficios a la relación entre profesionales y padres, así como
reflejos en la mejora de la seguridad infantil.
