Abstract
Background: Patient-reported outcome measures (PROMs) that assess pain, symptom, and physical function in individuals with Achilles tendinopathy are essential for effective treatment. Translation and cross-cultural adaptation, as well as the evaluation of measurement properties, are necessary to enable their use in practice. Objective: To translate, cross-culturally adapt, and evaluate the measurement properties of the TENDINopathy Severity assessment – Achilles (TENDINS-A) in individuals with Achilles tendinopathy. Methods: Adults with Achilles tendinopathy were included. Structural validity was assessed using confirmatory factor analysis, and internal consistency was evaluated using Cronbach’s alpha coefficient. Test–retest reliability was assessed through repeated administrations with a 5–14-day interval. Construct validity was analyzed by correlation with two other PROMs. Results: The TENDINS-A was translated and cross-culturally adapted into Brazilian Portuguese, maintaining semantic, idiomatic, and conceptual equivalence. Structural validity demonstrated good fit for a unidimensional 10-item model [Comparative Fit Index (CFI) = 0.979; Tucker-Lewis Index (TLI) = 0.971; Root Mean Square Error of Approximation (RMSEA) = 0.056; Standardized Root Mean Square Residual (SRMR) = 0.078]. Internal consistency was adequate (α = 0.836), and test–retest reliability was high for the total score (Intraclass Correlation Coefficient (ICC) = 0.88) with a standard error of measurement of 8.16% and a minimal detectable change of 22.62%. Construct validity was confirmed, with all seven hypotheses supported, showing strong correlations with the comparator instruments. Conclusion: The Brazilian Portuguese version of the TENDINS-A showed acceptable measurement properties to assess the severity of Achilles tendinopathy in the Brazilian context.
