dc.description.abstract | It is increasingly important to focus on the battle against bacterial resistance to antibiotics. The indiscriminate use of these drugs weighs on an already unbalanced equilibrium between strains resistant to the available medications and the difficult and slow process of creating new antibiotics. To mitigate these effects, it is mandatory that current antibiotics are able to maintain the lowest resistance profile
possible. The rationalization in the prescription has a predominant role in this goal, because the incorrect use can lead to selecting bacterial defense mechanisms. The current context, during the SARS-CoV-2 pandemic, in which several drugs were used in an attempt to reposition its function for the viral fight, became an additional complicating factor for the rational use of antimicrobials. When using Azithromycin for its immunomodulatory effect within a pathology that is essentially viral, the doors are opened for the increase of bacterial resistance to this medication and its drug class. There is plausibility to state that the indiscriminate use of this antibiotic, regardless of its intended function, increases the possibility of selecting resistant bacteria.
Azithromycin is a semi-synthetic compound discovered in the 1950s, based on the structure of erythromycin, the original drug of the class, and shares with it its mechanisms of action and has its main focus on inhibiting bacterial protein synthesis by binding to the 23S subunit ribosomal. Its means of resistance are well described and are important to understand the reasons for bacterial selection and can influence the search for new, more effective drugs. | eng |

dc.description.resumo | É cada vez mais importante o foco sobre a batalha contra a resistência bacteriana aos antibióticos. A utilização indiscriminada dessas drogas pesa numa balança já desequilibrada entre cepas resistentes as medicações disponíveis e o processo difícil e lento da criação de novos antibióticos. Para amenizar esses efeitos, é mandatório que os antibióticos atuais sejam capazes de se manterem com o menor perfil de resistência possível. A racionalização na prescrição tem papel preponderante nessa meta, pois o uso incorreto pode acarretar na seleção dos mecanismos de defesa bacterianos. O contexto atual, durante a pandemia do SARS-CoV-2, no qual diversas drogas foram utilizadas na tentativa de reposicionar sua função para o combate viral, entrou como um fator complicador a mais para o uso racional dos antimicrobianos. Ao utilizar a Azitromicina pelo seu efeito imunomodulador dentro de uma patologia que é essencialmente viral, abre-se as portas para o aumento da resistência bacteriana à essa medicação e sua classe medicamentosa. Há plausabilidade para afirmar que o uso indiscriminado desse antibiótico, independentemente de sua função buscada, aumenta a possibilidade de seleção de bactérias resistentes.
A Azitromicina é um composto semissintético descoberto na década de 1950, baseado na estrutura da eritromicina, a droga original da classe, e compartilha com ela seus mecanismos de ação e tem seu foco principal na inibição da síntese proteica bacteriana através da ligação com a subunidade 23S ribossômica. Tem seus meios de resistência bem descritos e são importantes para entender os motivos de seleção bacteriana e podem influenciar na busca de novas drogas mais efetivas. | por |
