dc.description.abstract | Cancer is considered a public health problem. Upon receiving the diagnosis of cancer, the individual may undergo changes in their habits and ways of living, which may compromise their health-related quality of life (HRQL) and their family members. The assessment of HRQoL and the resilience of oncology patients and their relatives can provide useful clinical information about the real impact on the sense of well-being and satisfaction with life and may favor the reduction of the impacts of clinical practice. The general objective was to evaluate the relationship between HRQoL and resilience and its association with self-esteem, hope, spirituality and anxious and depressive symptoms of cancer patients on chemotherapy and their relatives. This is a correlational cross-sectional study with a quantitative approach that was carried out at Instituto Amaral Carvalho. The sample consisted of 130 cancer patients on chemotherapy (QT) and 130 family members. In the individual interview, the following instruments were applied: Sociodemographic, Clinical and Health Characterization, EORTC-QLQ-C30 (this only in the (ERH), Herth Hope Scale (EEH), Pinto and Parent-Spirituality Scale (EEPPR), Rosenberg Self Esteem Scale (EAR) and Hospital Anxiety Depression Scale (HADS) . The analysis was performed by descriptive statistics, correlation analysis, mean and univariate and multivariate logistic regression. The project was approved by the Research Ethics Committee (CEP) of the Federal University of São Carlos (Opinion No. 1,680,639) and the Amaral Carvalho Hospital (Opinion No. 1,842,423). The results showed that patients presented a better perception of HRQoL in the "mental health" dimension of the SF-36 (70,98) and in the "social function" of EORTC-QLQ-C30. The worst perception of HRQOL was attributed to the "physical aspects" of the SF-36 (27,88) and the "fatigue" symptom of the EORTC-QLQ-C30 (29,15). Already the relatives indicated a better perception of HRQOL in "functional capacity" (86,69) and worse perception in "vitality" (66,04), both domains of the SF-36. A high level of resilience was observed between the patients (n = 63; 48.46%) and the relatives (n = 72; 55.38%). There was a relationship between HRQoL and the resilience of the interviewees, including patients and relatives with a high level of resilience presented better HRQoL. Patients and families with better HRQL and high level of resilience presented higher levels of spirituality, hope and self-esteem, and lower levels of anxious and depressive symptoms. Among the factors associated with the best HRQoL in patients with QT cancer, the lowest age, lower educational level, no use of medication, presence or absence of metastasis, no pain, high resilience, spirituality, hope and self-esteem, in addition to the low level of anxious and depressive symptoms. As for the relatives, the factors were male gender, not to help the family income, not to live with the patient, a high level of self-esteem and a low level of anxious and depressive symptoms. Among the factors associated with the high resilience of cancer patients in QT were the high level of hope and spirituality and the low level of depressive symptoms. For family members he found himself to be a practitioner of a religion and a high level of hope. dc.description.resumo | O câncer é considerado um problema de saúde pública. Com o recebimento do diagnóstico de câncer, o indivíduo pode sofrer mudanças em seus hábitos e modos de viver, podendo comprometer sua qualidade de vida relacionada à saúde (QVRS) e de seus familiares. A avaliação da QVRS e da resiliência dos pacientes oncológicos e de seus familiares pode fornecer informações clínicas úteis acerca do real impacto na sensação de bem-estar e de satisfação com a vida e podem favorecer a diminuição dos impactos sofridos com a prática clínica. O objetivo geral foi avaliar a relação entre QVRS e resiliência e sua associação com autoestima, esperança, espiritualidade e sintomas ansiosos e depressivos de pacientes oncológicos em tratamento quimioterápico e de seus familiares. Trata-se de um estudo correlacional, de corte transversal, com abordagem quantitativa, que foi realizado no Instituto Amaral Carvalho. A amostra foi composta por 130 pacientes com câncer em quimioterapia (QT) e 130 familiares. Na entrevista individual foram aplicados os instrumentos: Caracterização Sociodemográfica, Clínica e de Saúde, Medical Outcomes Study 36 – Item Short-Form Health Survey-SF-36, European Organization for Research and Treatment of Cancer - EORTC-QLQ-C30 (este apenas nos pacientes), Escala de Resiliência (ER), Escala de Esperança de Herth (EEH), Escala de Espiritualidade de Pinto e Pais-Ribeiro (EEPPR), Escala de Autoestima de Rosenberg (EAR) e “Hospital Anxiety Depression Scale” (HADS). A análise foi realizada por estatísticas descritivas, análises de correlação, comparação de média e de regressão logística univariada e multivariada. O projeto foi aprovado pelo Comitê de Ética em Pesquisa (CEP) da Universidade Federal de São Carlos (Parecer nº. 1.680.639) e do Hospital Amaral Carvalho (Parecer nº. 1.842.423). Nos resultados verificou-se que os pacientes apresentaram melhor percepção da QVRS na dimensão “saúde mental” do SF-36 (70,98) e na “função social” do EORTC-QLQ-C30. A pior percepção da QVRS foi atribuída aos “aspectos físicos” do SF-36 (27,88) e no sintoma “fadiga” do EORTC-QLQ-C30 (29,15). Já os familiares indicaram melhor percepção da QVRS na “capacidade funcional” (86,69) e pior percepção em “vitalidade” (66,04), ambos domínios do SF-36. Observou-se alto nível de resiliência entre os pacientes (n=63; 48,46%) e os familiares (n=72; 55,38%). Verificou-se relação entre a QVRS e a resiliência dos entrevistados, inclusive os pacientes e familiares com alto nível de resiliência apresentaram melhor QVRS. Os pacientes e familiares com melhor QVRS e alto nível de resiliência apresentaram maior nível de espiritualidade, esperança e autoestima e menores níveis de sintomas ansiosos e depressivos. Dentre os fatores associados à melhor QVRS dos pacientes com câncer em QT obteve-se a menor idade, a menor escolaridade, não fazer uso de medicamentos, presença ou ausência de metástase, não ter dor, alto nível de resiliência, de espiritualidade, de esperança e de autoestima, além do baixo nível de sintomas ansiosos e depressivos. Já para os familiares teve-se como fatores o sexo masculino, não ajudar na renda familiar, não residir com o paciente, alto nível de autoestima e baixo nível de sintomas ansiosos e depressivos. Entre os fatores associados à alta resiliência dos pacientes oncológicos em QT encontrou-se o alto nível de esperança e de espiritualidade e o baixo nível de sintomas depressivos. Para os familiares encontrou-se ser praticante de uma religião e o alto nível de esperança. Conclui-se que há relação entre QVRS e resiliência dos pacientes oncológicos em QT e seus familiares. | por |
