Prodepêutica pulmonar em pacientes com Síndrome Respiratória Aguda Grave por SARS-COV-2: um estudo comparativo

Introduction: Studies conducted in recent years have associated pulmonary auscultation and imaging patterns with the etiology of SARS-CoV-2 in patients with pneumonia. However, there is a need for studies to confirm and validate such diagnostic accuracy. Improving diagnostic suspicion with medical propaedeutics combined with imaging tests can bring clinical benefits in cases of difficulty in accessing the etiological diagnosis and even in the event of the advent of new strains that may be less detectable. Objective: This study seeks to associate the clinical and radiological propaedeutics of patients hospitalized with SARS with the etiological diagnosis, focusing on COVID-19. Materials and Methods: A cross-sectional observational study comparing pulmonary auscultation and radiological patterns on chest computed tomography (exposure factor) with the diagnosis of COVID-19 (outcome). To this end, the medical records of adult patients (≥ 18 years) hospitalized with SARS for at least 24 hours at the UFSCar University Hospital in 2020 were reviewed, with a confirmed or excluded diagnosis of COVID-19 by RT- PCR, with plain chest radiography or chest computed tomography in the 72 hours before or after admission. Pulmonary propaedeutics, radiographic and tomographic images were evaluated according to forms developed in the Redcap program. The calculated sample consisted of 42 clinical evaluations, 52 chest radiographs, and 96 tomographies. Patients were matched 1:1 by sex, date of admission, and age. Radiological images were analyzed by two evaluators. Results and conclusion: A total of 122 patients were included, with no significant differences between COVID-19 and non-COVID-19 patients in terms of pulmonary auscultation. Chest radiography was performed with the appropriate technique in only 53.8% of cases. The characteristics of plain chest radiography most associated with SARS-CoV-2 etiology were the presence of micronodules, involvement of the lower lateral region, and greater intensity of opacities. Chest computed tomography showed greater discrimination of etiological diagnosis in adult patients with SARS, with crazy paving opacities associated with COVID-19 and the finding of septal thickening not accompanied by ground glass and mediastinal adenomegaly associated with other etiologies. There was low agreement in the assessment of lung fields on plain chest radiography, which was better for assessing the mediastinum and costophrenic sinuses. There was moderate agreement in the assessment of tomographic images regarding the nature and distribution of lesions and the suspected etiological diagnosis.

This Final Course Assignment Thesis is part of the requirements set forth in the Pedagogical Program of the Medical graduation at the Federal University of São Carlos (UFSCar). It takes the form of a critical and reflective narrative about my personal journey - especially the academic one - throughout the past six years, primarily about my dedication to medical training. Although this is an academic assignment, reflecting on my experience as a medical student cannot be separated from the broader context of my life. After all, medicine is a profession rooted in caring for others, and both the person I am and the professional I am becoming are inseparable from the experiences and learning that have shaped me. This narrative is structured chronologically, starting with my entry into medical school, and is contextualized within the reality I lived at that time. It follows the academic progression through the three curricular cycles of the UFSCar Medical Program, with emphasis on the specificities of the course and how they resonated with my own way of learning, seeing and engaging with the world. Throughout, I critically reflect on each personal stage of this six-year journey, from 2020 to 2025, drawing connections between personal growth and professional development. The final section of this work addresses my current perspective on Medicine and the way I hope to practice it. It also touches on my expectations for life beyond the University and how I still believe there is space for emotion within professional life. I view as outdated the notion that Medicine must be purely rational, devoid of feeling. I’ve come to understand that empathy - an essential component of care - cannot exist without personal emotions. At the same time, I recognize the importance of practicing evidence-based Medicine, while striving to balance it with what I describe as “listening with the heart”. I am fully aware that this balance is not easily achieved; it is a continuous, intentional effort that requires continuous commitment. Caring for others also depends on caring for myself. Both are essential for providing meaningful, compassionate care. In the end, the very title of this work itself offers a fair summary of everything it represents.
Por trás de um diagnóstico: uma análise reflexiva sobre os ensinamentos do curso de medicina

This work presents a critical reflection on the journey of medical education, based on the author's personal experience. It aims to highlight the contrast between the initial idealizations of the medical profession and the practical realities of clinical work. Throughout the course, the student is faced with the daily routine of medical practice, which proves to be far from the collective imagination of heroism and social prestige. Medicine is understood as a continuous, demanding, technical, and human-centered activity that requires constant learning, emotional balance, empathy, and adaptability. The importance of understanding the patient as a unique individual is emphasized, recognizing that the experience of illness goes beyond pathophysiology and includes social, emotional, and contextual aspects. The text addresses the vulnerability of the Brazilian healthcare system, social inequalities, and the social determinants of the health-disease process. The academic experience during this period allowed the author to observe how factors such as housing, income, and access to information directly impact illness and healthcare. The persistent lack of health education is also discussed as a major challenge, making it difficult to promote healthy habits and patient autonomy. The role of the physician is highlighted as that of a guide, caregiver, and agent of change, even in the face of resistance. Extracurricular experiences in basic research and data analysis contributed to the author’s critical and scientific development, reinforcing the understanding of medicine as a technical, social, and investigative field. It is concluded that medical practice must be grounded in listening, science, and ethical responsibility, as a daily task of action, reflection, and constant improvement.

This paper aims to discuss the skills developed through problem-based learning in theoretical and practical settings, from the perspective of curricular development, integration with the São Carlos healthcare network, and the challenges posed during the undergraduate program. It also considers the adaptation of a fundamentally practical course to the quarantine context of the largest pandemic in recent history. The narrative will be presented chronologically in cycles that represent the different stages of medical training through the UFSCar medical course.
Do outro lado do leito: um relato reflexivo sobre a vivência da hospitalização antes da medicina

This work presents a reflective account of the author’s personal experience as a hospitalized patient in 2020, immediately before entering the Medicine course at the Federal University of São Carlos. The hospitalization occurred due to a ruptured ovarian ectopic pregnancy, requiring exploratory laparotomy and oophoroplasty, followed by care in the intensive care unit and general ward at the Centro Hospitalar Municipal Dr. Newton da Costa Brandão, located in Santo André, São Paulo. The account explores how experiencing a traumatic event and undergoing hospitalization impacted the author’s academic development and perception of life and the doctor-patient relationship. The reflection articulates concepts of life and death from both scientific and existential perspectives, integrating personal experience with the six years of medical training. This experience highlights how being “on the other side of the bedside” contributed to the development of humanistic competencies, empathy, and understanding of patient suffering and vulnerability. This work aims to demonstrate that experiences of illness can significantly influence the academic trajectory and professional development of medical students, reinforcing critical reflection on clinical practice.
Sobre o desenvolvimento da empatia ao longo de casos atendidos e de experiências vividas no curso de Medicina da UFSCar: uma análise reflexiva

The present final paper intends to analyze the journey of development of empathy during the six years of graduation in Medicine at the Federal University of São Carlos (UFSCar). The author addresses the ways in which empathy and the human exercise of medicine are taught and reinforced through experiences lived in the different cycles of the course, such as the use of narratives in activities of reflective practice, contact with diverse social realities, and patient consultations. The paper also relays specific clinical cases that have affected this trajectory in different aspects. The paper concludes that the UFSCar course is efficient in training professionals with adequate technical knowledge and empathy, enabling them for the excellent practice of medicine.

This Final Coursework consists of an experience report in a narrative format about the six-year journey through undergraduate medical training at the Federal University of São Carlos (UFSCar) between 2020 and 2025. The objective is to analyze the difficulties, learnings, and transformations that shaped the construction of my professional identity. The trajectory is initially marked by the impact of the COVID-19 pandemic, which imposed a distance learning model for the first two years. This phase generated frustration due to the absence of practical activities, insecurity with clinical skills, and the need to develop resilience and proactivity. The return to in-person learning from the third year onward represented a "turning point," allowing for the rediscovery of the passion for medical practice, despite the reality shock of dealing with the complexity of real patients. The fourth year was a period of consolidating confidence and maturing clinical reasoning, with the development of the concept of "longitudinal care". The internship (fifth and sixth years) is described as a "culture shock" between hospital medicine and primary care, and as the period of definitive discovery of the vocation for Internal Medicine, motivated by the intellectual challenge of complex cases. Extracurricular activities are presented as a strategic tool that evolved from online exploration to immersive internships that solidified the choice of the future specialty. It is concluded that, despite its flaws and challenges, the training was paradoxically effective in forging a resilient professional with a profound clarity about his path in medicine.
Livraria “independente” na era digital: um estudo de caso da Barsa Livros como rugosidade

In order to formulate its basic hypothesis, this research starts from the following question: does Barsa Livros, an independent bookstore, present itself as an element of friction [rugosidade] within the dynamics of vertical and horizontal relationships of the social organization? Barsa Livros is an independent bookstore in Ribeirão Preto, state of São Paulo, Brazil. It began its activities in 2020, initially operating digitally and with a sales system based on orders. As it developed, it adopted a hybrid approach: dealing with printed books and building its inventory; participating in fairs held in various cultural spaces; and, subsequently, also establishing a physical location, where its activities now take place and where its entire collection is housed. Through a case study, we propose to analyze the publishing market – notably the bookselling sector – in its current historical particularity, with the presence of digital technology and the power exerted by the use of information, presented within the empirical framework of John B. Thompson (2021) and through Bourdieu's (2018) field theory, in order to define the publishing and bookselling field and position the studied bookstore within it. Furthermore, in light of José De Souza Muniz Júnior's thesis (2016), we will study the position of "independent" within this publishing field, as well as the polysemy of the term and its possible characteristics in order to understand the possibility of Barsa Livros presenting itself as an independent bookstore in this field. Subsequently, using Milton Santos' geographical theory (2023), the dynamics of verticalities and horizontalities that compose the banal space will be analyzed in order to verify whether Barsa Livros presents itself as an element of friction within it. Finally, we will test the hypothesis that this bookstore functions as a medium and materializes this friction in space through the methodology of materialized organization and organized matter (OM/MO) as seen in Régis Debray (2000), in addition to analyzing the bookstore's collection as a technema: a technique that makes it point to reticular and/or contiguous organizations, as well as, in its double-body, relate to hegemonic and/or popular sensibilities (Santos, 2022).
Experiências de mulheres no pós parto em situação de vulnerabilidade usuárias de implante de etonogestrel

In Brazil, many unplanned pregnancies occur during the postpartum period, causing harmful consequences for women, children, and families. The use of an appropriate contraceptive method in this group aims to mitigate this unfavorable situation. In this sense, long-acting contraceptive methods (LARC) appear to be quite satisfactory options due to their specific characteristics, especially the etonogestrel subdermal implant. This study aims to evaluate the experiences of vulnerable postpartum women using the etonogestrel implant in a city in the interior of the state of São Paulo. The study has an observational, cross-sectional, descriptive/quantitative profile of postpartum women who have been using this method for at least three months. The survey revealed that the social profile of most participants was between 17 and 19 years old (24.1%), mixed race (41.4%), high school graduates (31%), and the majority were single (51.7%), and evangelical (27.6%). Regarding parity, the vast majority of women already had at least one child (93.1%). Regarding vulnerability, the most common conditions were alcohol and/or drug use (28%), adolescents with children (24%), and mental and cognitive disorders (20%). Regarding previous contraceptive methods, we found that more than half of the women were already using some form of contraception before implants (51.7%), with the main choices being oral contraceptives and male condoms. Breastfeeding was also common among study participants (82.7%), with the duration ranging from 1 month to 2 years and 4 months. The implant was considered satisfactory by most participants (55.2%), with efficacy (65.5%) and convenience (34.4%) being the main positive aspects reported. Regarding bleeding patterns, 41.3% reported patterns considered favorable (infrequent/regular/amenorrhea). Although frequent and prolonged bleeding was the main complaint (55.1%), and prolonged bleeding predominated between 6 and 24 months, with a tendency to improve after this period, 75% of users were satisfied with their bleeding pattern, and almost half (48.2%) considered the reduction in menstrual flow positive. The data obtained allow us to conclude that there is a trend toward a high level of overall satisfaction among users of the etonogestrel implant, reinforcing its efficacy, convenience, and suitability, especially in vulnerable settings.
